ZynxCarebook’s New Transitions of Care Content Enhances Moving of Patients to New Settings
Most relevant evidence-based best practices are accessible to workflows of all care team members for immediate care delivery standardization
LOS ANGELES,CA – August 10, 2015 -– Zynx Health™, the market leader in providing evidence- and experience-based clinical improvement solutions, today announced it has enhanced its ZynxCarebook™ mobile care coordination solution with new evidence-based transitions of care content. Developed specifically by Zynx Health, this content provides the most relevant care guidance to further improve care team coordination and the patient’s healthcare experience in moving to new care settings.
“Patients are at their most vulnerable when transitioning from the hospital to the nursing home, outpatient practice, or other facility,” said John Chang, M.D., senior director of content for Zynx Health. “Our clinicians are experts at organizing the available wealth of evidence-based content that often goes unused but can dramatically ease those changeovers. By adding our transitions of care content to ZynxCarebook, we can make the best evidence-based information accessible to the workflows of all care team members working across all care settings, thus assuring the highest patient safety adherence.”
Zynx Health’s team of clinicians accumulates and appraises peer-reviewed research, national guidelines, and performance measures. These experts classify the evidence sources based on a rigorous, reproducible, and tested methodology. This meticulous editorial process drives Zynx Health’s transitions of care content to comprise the most relevant evidence-based clinical care guidance and best practices from evidence-based transitions of care models such as Project RED® and Project BOOST®.
Healthcare organizations can improve outcomes by utilizing transitions of care content embedded in ZynxCarebook that not only provides evidence-based clinical guidance but also enables the optimization of workflow around the discharge and transition process. Ultimately, the combination of improved communication with care guidance–enabled workflow allows standardized care delivery that is based on the most current evidence-based best practices. With ZynxCarebook, along with our industry-proven, customer-driven implementation and methodology–armed with this inclusive, richer medical knowledge–the care team providers can optimally improve financial and clinical outcomes beyond the limited use of an individual evidence source.
Organizations using ZynxCarebook for their mobile care coordination needs have demonstrated significant improvements in their care transitions, reductions in readmission rates, and decreases in lengths of stay. These outcomes not only lower costs, but are also consistent with the goals outlined by government healthcare-improvement initiatives such as the Value-Based Purchasing Program, the Hospital Readmission Reduction Program, and the Delivery System Reform Incentive Payment Program.

Presbyterian Healthcare Services Taps Zynx Health’s Knowledge Analyzer to Standardize Latest Clinical Content Enterprise-Wide
LOS ANGELES, CA – August 3, 2015 — Zynx Health™announced today that Presbyterian Healthcare Services, New Mexico’s only private, not-for-profit healthcare system, has entered into an agreement to deploy Knowledge Analyzer™ to standardize their clinical decision support (CDS). Presbyterian will use the solution to compare the most current evidence-based intelligence against their existing content and identify opportunities that have an impact on outcomes, quality performance measures, and national care guidelines.
The Albuquerque-based health system encompasses eight hospitals serving more than 48,000 patients annually as well as 30 primary care and specialty clinics and nine urgent care facilities across the state. By evaluating their CDS using Knowledge Analyzer, Presbyterian’s 770 physicians in 50 specialties can stay current at the point of care on vital interventions affecting five key outcomes: mortality, length of stay, admissions and readmissions, healthcare costs, and hospital-acquired conditions.
“New evidence-based medicine is introduced almost daily, and regulations and national standards change frequently,” said Kevin Maben, MD, associate chief medical information officer at Presbyterian. “As one of the largest providers in New Mexico, we have a commitment to our patients to stay on top of these developments to ensure the very best, safest, and most affordable care. With Knowledge Analyzer, our clinicians can review the latest clinical knowledge while calling out interventions affecting clinical and financial performance.”
Knowledge Analyzer analyzes content from any system, whether homegrown or from another third-party vendor, and is compatible with all electronic health record vendors. As a result, Presbyterian can facilitate the content updates within the Epic electronic health record without having to recreate the current content.
“Presbyterian Healthcare Services is responsible for tens of thousands of lives in New Mexico,” said Bertina Yen, MD, MPH, executive vice president, product group at Zynx Health.
“They are clearly committed to delivering the best care in the safest way at the lowest reasonable cost. We are excited to support their numerous provider facilities with reviewing, analyzing, and updating their CDS against the latest evidence-based content through Knowledge Analyzer.”
Dr. Maben added, “We selected Knowledge Analyzer for its ability to analyze our clinical documentation down to the line item intervention level and so we could perform evidence updates within Epic. The automated analysis saves time, speeding delivery of approved evidence to clinicians and reducing the risk of errors and incomplete or missing content.”
Knowledge Analyzer displays the gaps in content that do not meet present care standards. It also highlights missing or outdated items that can have an impact on clinical outcomes including national quality measures and guidelines. Clinicians can further discuss and reach consensus on the evidence recommendations to reduce care variability and take appropriate action.
