Cookeville Medical Center Selects Allscripts for Chronic Care Management (CCM) Program
Solution Enables Improved Coordination & Reimbursement of Care for Patients With Chronic Conditions
CHICAGO,IL – August 25, 2015 (GLOBE NEWSWIRE) — Cookeville Medical Center in Cookeville, Tenn., selected Allscripts(Nasdaq:MDRX) Chronic Care Management (CCM) Program to more easily manage and improve care for patients with chronic conditions while increasing Centers for Medicare & Medicaid Services (CMS) reimbursements.
Cookeville Medical Center is an existing Allscripts Professional EHR™ client that selected the CCM program to improve engagement with patients for better management of chronic diseases. The CCM program enables Cookeville to offer patient-centered care plans with limited impact to existing operations.
The Allscripts CCM program features services from Allscripts’ Professional Physicians business unit to assist care providers in reducing the complexity of caring for chronic patients, including advisory services, patient outreach, practice and claims management.

Solution Helps to Enable Practice to Rollout Proactive Outreach to Hundreds of Patients With Chronic Conditions & Coordinate Medicare & Medicaid Reimbursement
CHICAGO,IL – September 1, 2015 (GLOBE NEWSWIRE) — Villa Medical Clinic in Newburg, Ore., selected Allscripts (Nasdaq:MDRX) Chronic Care Management (CCM) Program to establish ongoing monthly outreach to hundreds of patients who suffer from two or more chronic conditions without adding staff or increasing workloads for current employees.
Villa Medical Center has served its community for more than 50 years and currently provides care services with one physician, three physician assistants and a team of nurses and support staff. The practice sought a way to simplify the rollout of care for patients with chronic conditions through the Centers for Medicare and Medicaid services (CMS) reimbursement program. The practice is in the process of transitioning to Allscripts Professional EHR™ from which it will be able to identify Medicare and Medicaid patients with chronic conditions who could benefit from increased care.
